rbd: allocate image object names with a slab allocator

The names of objects used for image object requests are always fixed
size.  So create a slab cache to manage them.  Define a new function
rbd_segment_name_free() to match rbd_segment_name() (which is what
supplies the dynamically-allocated name buffer).

rbd: clear EXISTS flag if mapped snapshot disappears

This functionality inadvertently disappeared in the last patch.

Image snapshots can get removed at just about any time.  In
particular it can disappear even if it is in use by an rbd
client as a mapped image.

The rbd client deals with such a disappearance by responding to new
requests with ENXIO.  This is implemented by each rbd device
maintaining an EXISTS flag, which is normally set but cleared if a
snapshot disappears.

This patch (re-)implements the clearing of that flag.

Whenever mapped image header information is refreshed, if the
mapping is for a snapshot, verify the mapped snapshot is still
present in the updated snapshot context.  If it is not, clear the
flag.

It is not necessary to check this in the initial probe, because the
probe will not succeed if the snapshot doesn't exist.

dma:of: Use a mutex to protect the of_dma_list

Currently the OF DMA code uses a spin lock to protect the of_dma_list from
concurrent access and a per controller reference count to protect the controller
from being freed while a request operation is in progress. If
of_dma_controller_free() is called for a controller who's reference count is not
zero it will return -EBUSY and not remove the controller. This is fine up until
here, but leaves the question what the caller of of_dma_controller_free() is
supposed to do if the controller couldn't be freed.  The only viable solution
for the caller is to spin on of_dma_controller_free() until it returns success.
E.g.

do {
ret = of_dma_controller_free(dev->of_node)
} while (ret != -EBUSY);

This is rather ugly and unnecessary and none of the current users of
of_dma_controller_free() check it's return value anyway. Instead protect the
list by a mutex. The mutex will be held as long as a request operation is in
progress. So if of_dma_controller_free() is called while a request operation is
in progress it will be put to sleep and only wake up once the request operation
has finished.

This means that it is no longer possible to register or unregister OF DMA
controllers from a context where it's not possible to sleep. But I doubt that
we'll ever need this.

Also rename of_dma_get_controller back to of_dma_find_controller.

dma_spec of_node if no DMA controller matching the of_node could be found. This
patch fixes it by always calling of_node_put().

Command Window value from the CQE was used to calculate the
max_cmdsn for that session.The command window value extracted
for SKH-R adapter was not proper. The value was extracted from
BE adapter completion event. Fixed the issue by getting the
cmd_wnd value from SKH-R CQE.

The exp_cmdsn and max_cmdsn values were not converted to BE format
before calling the __iscsi_complete_pdu(). Fixed the issue of converting
to BE format.

however the above routine declares the ret variable as static!
Thus there is only one instance of this variable!
When this routine is called from more than one thread of execution,

When target send a NOP-IN with valid TTT, driver issues a NOP-OUT
and the task was not freed from driver. The task list available for
the session used to run out, and as no more task list were available
no more iSCSI commands were exchanged on that session.
This patches fixed the issue, by calling iscsi_put_task.

The mgmt_hba_attributes structure declared was not proper and
because of that the FW response returned for the MBX_CMD was not
matching. This issue went unnoticed as mgmt_hba_attribs structure
members were never used in the code path.

This fix of displaying the FW version had to change the mgmt_hba_attrib
structure also. The latest driver will also work with the older FW as
the issue was in the driver declaration.

ipc_schedule_free() can do vfree() directly now

Commit 32fcfd40715e ("make vfree() safe to call from interrupt
contexts") made it safe to do vfree directly from the RCU callback,
which allows us to simplify ipc/util.c a lot by getting rid of the
differences between vmalloc/kmalloc memory.

net: Restore NETIF_F_* bit ordering.

Commit 8ad227ff89a7 ("net: vlan: add 802.1ad support") added some new
NETIF_F_* features bits, but it added them in the middle of existing
values.

Userland depends upon the flag bits via the per-netdevice 'flags' sysfs
file.

So restore the previous ordering by adding the new flags at the end.

incorrectly assumes that abort failed and performes recovery by issuing

FIP VLAN discovery discovers the FCoE VLAN that will be used by all other FIP
protocols as well as by the FCoE encapsulation for Fibre Channel payloads on
the established virtual link. One of the goals of FC-BB-5 was to be as
nonintrusive as possible on initiators and targets, and therefore FIP VLAN
discovery occurs in the native VLAN used by the initiator or target to
exchange Ethernet traffic. The FIP VLAN discovery protocol is the only FIP
protocol running on the native VLAN; all other FIP protocols run on the
discovered FCoE VLANs.

If an administrator has manually configured FCoE VLANs on ENodes and FCFs,
there is no need to use this protocol. FIP and FCoE will run over the
configured VLANs.

An ENode without FCoE VLANs configuration would use this automated discovery
protocol to discover over which VLANs FCoE is running.

The ENode sends a FIP VLAN discovery request to a multicast MAC address called
All-FCF-MACs, which is a multicast MAC address to which all FCFs listen.

All FCFs that can be reached in the native VLAN of the ENode are expected to
respond on the same VLAN with a response that lists one or more FCoE VLANs
that are available for the ENode's VN_Port login. This protocol has the sole
purpose of allowing the ENode to discover all the available FCoE VLANs.

Now the ENode may enable a subset of these VLANs for FCoE Running the FIP
protocol in these VLANs on a per VLAN basis. And FCoE data transactions also
would occur on this VLAN. Hence, Except for FIP VLAN discovery, all other FIP
and FCoE traffic runs on the selected FCoE VLAN.  Its only the FIP VLAN
Discovery protocol that is permitted to run on the Default native VLAN of the
system.

We are working on moving this feature definitions and functionality to libfcoe
module. We need this patch to be approved, as Suse is looking forward to merge
this feature in SLES 11 SP3 release.  Once this patch is approved, we will
submit patch which should move vlan discovery feature to libfoce.

Commit 0c2c99b1b "memory hotplug: Allow memory blocks to span
multiple memory sections" introduced a weak memory_block_size_bytes()
function which can be used to set the size of a memory block as
seen in sysfs.
Provide an s390 specific override which makes sure that each
memory block has at least a size of 256MB or the increment size of
of a memory increment, whatever is larger.
This way we can make sure that the number of memory sysfs objects
doesn't explode for very large memory configurations.

The current memory detection loop will detect all present memory of
a machine. This is true even if the user specified the "mem=" parameter
on the kernel command line.
This can be a problem since the memory detection may cause a fully
populated host page table for the guest, even for those parts of the
memory that the guest will never use afterwards.

So fix this and only detect memory up to a user supplied "mem=" limit
if specified.

When in kdump mode the kernel may access only the first couple of
megabytes for execution, the rest contains the dump. However
the size of the bitmap used by the bootmem allocator was calculated
for the whole amount of memory of the machine.

For very large machines this can lead to the situation that the kdump
kernel will not come up because not enough memory is available.

So fix this and calculate the size of the bitmap only for the piece
of memory that the kdump kernel actually uses.

Call reserve_oldmem() before setup_memory_end() so that the memory_chunk
array already has been updated with respect to oldmem chunks.
Afterwards setup_memory_end() will ignore those chunks.

s390/kvm: Change the virtual memory mapping location for Virtio devices

The current location for mapping Virtio devices does not take
into consideration of the standby memory. This causes the failure
of mapping standby memory since the location for the mapping is
already taken by the Virtio devices. To fix the problem, we move
the location to beyond the end of standby memory.

s390/zcore: calculate real memory size using own get_mem_size function

The zcore device driver makes use of the global visible real_memory_size
variable with its odd semantics.
Since the zcore device driver already has code in place which calculates
the memory size at module load time, use that code to calculate the current
memory end value.

One user less of the odd real_memory_size variable.

s390/mem_detect: fix lockdep irq tracing

When disabling and enabling interrupts we must tell lockdep.
So use local_irq_save()/restore() to disable and enable interrupts.
The DAT disabling/enabling get handled separately now.
Note: we may not call trace_hardirqs_on() with DAT disabled, since
the generic code may access vmalloc'ed data structures.

KVM: PPC: Book3S: Add API for in-kernel XICS emulation

This adds the API for userspace to instantiate an XICS device in a VM
and connect VCPUs to it.  The API consists of a new device type for
the KVM_CREATE_DEVICE ioctl, a new capability KVM_CAP_IRQ_XICS, which
functions similarly to KVM_CAP_IRQ_MPIC, and the KVM_IRQ_LINE ioctl,
which is used to assert and deassert interrupt inputs of the XICS.

The XICS device has one attribute group, KVM_DEV_XICS_GRP_SOURCES.
Each attribute within this group corresponds to the state of one
interrupt source.  The attribute number is the same as the interrupt
source number.

This does not support irq routing or irqfd yet.

The full mcpm layer is not likely to be relevant to v6 based
platforms, so a multiplatform kernel won't use that code if booted
on v6 hardware.

specify armv7-a explicitly for that code.

OMAPFB: defer probe if no displays

omapfb requires the panel drivers to have been probed when omapfb is
initialized. omapfb does not support insertion of new panels after its
probe. This causes a problem in case omapdss or the panel probes have
been deferred due to EPROBE_DEFER error, as omapfb won't find any
displays.

As a quick fix, this patch changes the omapfb probe so that if omapfb
does not find any displays, it'll return EPROBE_DEFER. This is not
perfect, as with a board with no displays, omapfb will get deferred
forever. Also, if the board has multiple displays, but only some of them
have been probed, omapfb will start and leave the unprobed displays out.
